How did we get here?

****Spoiler Alert**** The second season finds The Sniffer in new digs, his would-be girlfriend in an institution, & him shot. There are too many unanswered questions in this episode & I'm now in ep 3 still hoping for some answers. Why does he have a new place?? How did the Dr end up institutionalized (was it PTSD from being held hostage?? That few minutes with her Dr just wasn't enough of a transition.)? And was there a note telling us that after him going to the hospital that everything that followed was before the shooting? We need some hint of time passing in this episode.
