Okay straight off the bat I would say to anyone about to read my review it does contain SPOLIERS so continue on at your own peril. If your a fan of indie horror I would say to give this one a try. After reading all the other 27 reviews before mine not one person seems to have interpreted this film as I did, which is fine but I did find that a bit odd, so I thought I'd weigh in with my 2 cents.

Right so basically the premise of the film. Young guy James plays music, takes drugs with his friends silly pranks and highjinks occur. Sara meets James and drugs aren't really her thing but she ends up doing trips (hallucinogens) acid and mushrooms I believe and learning of an urban legend named the toad road and becoming a bit obsessed with it.

The toad road is supposedly where u take a walk through the woods on acid and keep waking through 7 imaginary gates you can only see on drugs and each gate is supposed to represent a different circle of hell. James isn't really keen on the idea but he goes along anyway, to keep sara company and to look after her as she's relatively new to drugs.

Now from what I deduced before James passes out face down in the leaves and the dirt, you see Sara and him waking through the woods waiting for it to get dark and the gates to appear. At one point you see Sara stop walking as James continues. Now at this point James looks back to where Sara is and there's this fuzzy/distorted portal type doorway right in front of her.

Sara does a voiceover telling you all about the different gates and what to experience from each one about 5 minutes before this scene, I took it to mean that she walked though the 5th doorway.

I think it's the 5th as she states in her voiceover that in the previous doorways time has no meaning and things won't make sense and no one will try to put you back on the right path and eventually you will just lose yourself. You'll spiral out of control, everyone around you will fade away and die, you won't be able to sleep etc ect, so after he wakes up in the woods I just assumed that infact what happened to James was that he was unable to find the doorway Sara did. Once he awakens he briefly looks for Sara, can't find her gives up on this idea far too quickly, seems quite confused and goes back to his apartment.

He arrives at his apartment to find his key doesn't fit, he also later learns he doesn't live there anymore. He gets informed by one of his friends that both him and Sarah have been missing for months, all his friends have moved away, Sara's stil missing and he's become a person of interest in her disappearance and is later questioned by the police.

So essentially when he wakes up in the forest everything your seeing on film is just a distorted version of reality, he's become lost, not being able to find the gate Sara did and everything your now seeing is all just a world that's been created for him as he wasn't strong enough to reach the gate.

Anyway this is just my take and I could be wrong but I think it explains the film a bit better, but like I said this was just what I took from watching this film.
