When Walker fights with Karli he throws the shield, but it immediately reappears in his hand.
When John Walker was assembling his homemade shield in the previous episode, the center part (blue circle with a silver star) was seen surrounded by a white circle, while he had spray-painted two larger circular pieces (which have yet to be welded together) red. The Captain America shield features a blue center surrounded by a red circle, then white/silver, then another red circle for the outer edge. The color pattern has been corrected for this episode, but in doing so has created a continuity error.
The number of holes in the knees of Karli's pants changes repeatedly between shots/scenes. Sometimes both knees are torn out, other times just the left knee is torn out, and in some shots the knees are completely intact.

Factual errors

The statue of Isaiah Bradley has a US insignia on both upper lapels and infantry insignia on both lower lapels. It should be a US insignia on the right upper lapel and the infantry insignia on the left upper lapel.
A flying helicopter is too noisy to be heard speaking on a mobile phones with speakers on and the pilot cannot hear the interlocutor because he is wearing muffling headphones.

Miscellaneous

In the car bomb scene, it's showed 3 flag smashers entering the vehicle, after that, the car explodes. On the next scene, Zemo is listening the news on the radio and the reporter says: "Four members of the radical group The Flag Smarshers were killed today in a car bomb attack as they were being transferred to a high security prison(...)"
When Bucky, Sam are in the station after Bucky's arrest, bucky can be seen with the 2 cops behind Sam and Dr Raynor while they are being approached by John but the next scene is what shows the cops and bucky exiting the locked door to go into the lobby.

Audio/visual unsynchronised

When Bucky knocks on Mr. Nakajima's door, the sound is that of a hard object striking against wood, as if he was hitting it with bare knuckles. Bucky is wearing leather gloves, which would soften the sound considerably.

Plot holes

Sharon Carter arrives in New York at the same time as Bucky and Sam, with Sam saying she is there because he called in a favor. She was previously in Madripoor, an island nation in Southeast Asia. She would had to have smuggled herself from Asia to New York, probably via illegal channels (as she is a wanted criminal), in the same amount of time it took Bucky to walk to the GRC building from his apartment and Sam to fly to New York from Louisiana.
Sam Wilson makes it from his family home in Delacroix, Louisiana to Manhattan in a matter of minutes. Those two cities are more than 1300 miles apart.
Sharon is revealed to be entering the GRC building armed because she sets off the metal detector. Bucky walked through the detectors ahead of her and did not set them off, even though he has a fully robotic arm.

Character error

Karli insinuates that Bucky has never fought for anything bigger than himself. Buck fought the Nazis in World War II and fought Thanos' army both before and after the blip for the fate of the universe.
