When a patient suffering from anorexia can't gain the necessary weight needed to survive heart surgery, Claire suggests they perform an experimental surgery which Melendez strongly opposes. Shaun worries Glassman's refusal to walk the hospital floor will keep him from being discharged and tries to persuade him to ambulate. Meanwhile, Shaun and Lea change the terms of their current relationship.

Lea stayed more days at Shaun's after their uneasy friendship ended up with a karaoke last night. Before going to work, Lea tells Shaun she hopes not to live together in a new apartment because she only wants to keep Shaun as a friend, not living in a couple-like way.

Shaun finds an interesting case to look after as he spots a sample of fizzy urine. It belongs to a patient, Wade, who has sore in the right knee joint. He barely noticed his fizzy urine and he has a record of gastric bypass surgery in the past to quickly reduce his overweight. Shaun concluded he has a severe health problem and proposes cystoscopy.

A woman, Louisa, rushed to the hospital after experienced HVT along with multiple loss of consciousness. She is in terribly weak condition due to anorexic. Melendez decides Louisa needs surgery to fix her heart valve failure which common to a person with malnutrition.

Glassman is back and Shaun regretted he chooses to play a card in the bed instead of doing physical therapy for post-operation treatment.

The cystoscopy procedure successfully discovers the explanation for the presence of bubbles in Wade's urine. He is diagnosed with a Crohn's disease, thus his gastric bypass must be reversed. Wade does not mind it unless the doctors help him to deceive his husband about the reverse. Shaun offers a clever idea, a single abdominal port, which allowing two surgeries, bladder and stomach, conducted through a single incision.

Louisa's malnourishment state won't allow the surgery unless her body is supplied with calories. Louisa agrees to eat in a normal way but she takes the food sluggishly. She gives up then asks Morgan to insert NG Tube to directly carry food to the stomach.

Shaun meets Dr. Blaze to discuss Glassman's reluctance to do physical therapy. She advises if the "stick" (punishment) didn't work then a "carrot" (reward) is worth trying. Later, Shaun bought a chocolate Muffin, Glassman's favorite, and put that at the nurse station to motivate Glassman to walk. But Shaun fails twice, Glassman doesn't want to move and someone eat the muffin.

Louisa is vomiting after the NG Tube is inserted for an hour. She used to discomfort with any treatment with the tube. Claire suggests deep brain stimulation (DBS), based on a small experiment in the UK, to radically changes Louisa's perception about food. Melendez opposed the idea since it comes from a small sample of research with only 50% success and has side effects of personality changing. Melendez instructs to do the surgery anyway.

Lea reclaims Shaun's apartment rent deposit but Shaun is not happy with it. He acquired the apartment back and showing the key to Lea in the next morning. He says he doesn't mind with any annoyance that may come caused by Lea's normal routine. He just wants to live good and happy, although Lea refuses it.

Wade's marriage is at risk as his husband finally knows the truth about his medical condition. In the middle of the couple bicker, Shaun identifies Wade's bowel is bleeding and needs immediate surgery.

Louisa's husband meets Melendez to refuse his plan to continue the heart surgery and asking him to do the deep brain stimulation instead. This case finally ended up in an internal review board. Claire presented the experiment result and Louisa describes her hopes for this treatment. By margin 2-1, Claire's proposal is approved and Louisa DBS went success.

Shaun comes back to Marina, looking for any solution. Marina thinks Glassman needs a true friend to be with him during tough times. Shaun seems like having a plan. Later, Debbie visits Glassman with a muffin, and apparently, Shaun's plan works. Shaun also realizes he is not Glassman's top friend.

Park meets Wade's husband to inform the surgery went well. The husband is staring at a photo of Wade's and surprised knowing Wade had obese and this surgery makes the condition back.

Claire reports to Melendez that Louisa starts eating and will be ready for valve surgery in the next couple of weeks. Melendez appreciates but will not allow Claire to be with him in every single future surgery including this one. He feels irritated by Claire's moves which undermine his past decision to refuse the DBS.

Shaun goes home and he is delighted after Lea says is OK to be his roommate.
