Christian's regular assignments as an emergency doctor are now pushing him to the limits of his endurance. To be able to meet the needs of his patients during the day, he continues to take tranquilizers. An elderly patient, Elfriede Fischer, presents to the practice with acute heart problems. She urgently needs a new heart valve, but she is terrified of the procedure. Since transfer to an outside clinic is not an option, Michael suggests a modern, gentle surgical procedure in Eisenach, which leads to a clash with Professor Bender. Bender insists on a traditional, less expensive procedure for economic reasons. When Michael nevertheless decides in the patient's best interests, thereby overriding his boss, he is threatened with a warning. At the same time, Christian and his colleagues Lisa and Timo are investigating a mysterious case: The young gymnast Melanie is about to compete in an important competition and is complaining of chronic exhaustion. However, her values are within the normal range. Christian finally discovers a possible cause for Melanie's symptoms, which is much more serious than suspected. Piwi has still disappeared without a trace after the argument with Christian. Anke and the Kleists wait in vain for a message from him and gradually begin to worry. Lisa manages to find Piwi and get through to him. When Piwi tries to talk to Christian, he reacts surprisingly harshly: He's fed up with his son's antics and insists on a decision that will affect the entire practice. Anna struggles with her dubious mandate. She seeks advice from Christian. His medical analysis of the case makes her seriously doubt her client's innocence. Meanwhile, Lisa tries to avoid Timo.
