- Over the course of the game, the player-character encounters both human and supernatural enemies which must be defeated in order for the player to progress through the main storyline.
- The protagonist uses his fists to punch enemies, and wields weapons such as swords, spears, axes, hammers, muskets and bows, as well as magical attacks, to combat armored knights, militia infantrymen, ninjas and demons.
- Combat involves the player-character slashing or hitting the enemy to inflict damage onto them.
- Enemies can be decapitated or dismembered when an attack is focused on a specific part of their body.
- For example, firing an arrow at an enemy's head will result in decapitation and a spray of blood from the severed stump of the neck.
- Blood effects occur frequently during combat and will stain nearby surfaces, including the player-character's clothing and weapons.
- The depictions of violence are realistic, coupled with frequent blood effects.
- Overall, various depictions of realistic violence, in forms such as killing, maiming and/or causing other serious injury to humanoid characters is featured, but the violence itself is not sadistic, cruel and/or abhorrent.

- This game has a lot of similarities to Dark Souls. Because of this the game can be rather punishing and difficult with its combat and can be possibly intense or frustrating for some players.
- 'Nioh' is an action role-playing game that revolves around William, a swordsman who has traveled to feudal Japan in search of a mysterious man. However, he finds himself fighting against demons that have plagued the country.
