During the task force's arrival on the prison facility, Agent Navabi is shot in the leg by Braxton's team. When the team arrives back home, and they exit vehicles, Agent Navabi is no longer injured.
When Red confronts Kotiopulos at his home, Braxton is hanging dead from a chandelier. In one shot during the conversation that frames Dembe, Kotsiopulos, Braxton, and Red, it is clearly not Ron Perlman who is hanging from the chandelier, but a body double whose face isn't even closely resembling Perlman's.
