Judith looks almost exactly the same age in flashbacks as she does at the present time, even though the flashbacks take place many months earlier, when she would have been just a few weeks old.

Factual errors

As the group leave what is left of Terminus Rick marks a sign, "No Sanctuary". The group is LEAVING Terminus, the sign should be facing the other way.
Right before Rick attacks a Termite and takes his AK-47 away the man and the others he is with are seen firing their rifles from the hip, with the barrels aimed at a downward angle. This would cause them to hit the walkers in the legs, not hit them in the chest and head as shown.

Revealing mistakes

Towards the beginning of the episode, around 02:45 (2 minutes, 45 seconds), a guard can be seen holding up his right hand to his mouth as if he is "talking" into a walkie talkie/radio, but he is not actually holding anything.
When everyone's reaction in the slaughter room from the explosion Adam Boyer who plays Bat Smock Man is the only one who stands completely still as everyone else falls back or forward.

Miscellaneous

At around 38:40 when Tyreese is walking out of the cabin with Judith he closes the door behind him. A few minutes later when he's hugging Sasha you can see the door behind them and it's wide open.

Plot holes

Carol doesn't know where all her people are located, nor could she know how large such an explosion would be. From her perspective, any explosion and resulting fire could kill them as well.
Carol remembers the walker gut camouflage trick, but bizarrely (considering her interactions with Lizzie) doesn't seem to remember significant walker abilities/thinking behavior that surrounded the gut use in the first place. - A walker using a rock to break glass being just one example.

Character error

As Rick is telling Gareth about the weapons in the bag he had hidden, he mentions a .44 magnum, he's referring to his signature sidearm, which is a Colt Python, however, this weapon was only ever made in a .357 magnum caliber.
