Continuity

In the beginning of the movie, Hogan is a janitor and walks in backwards to Bobs office, he takes the tray of pastries from the table and dumps them in his cart but after he tries to tag Bob the tray is back on the table.
At around 53 minutes in, Chili is on foot chasing someone through a forest. His hands and face are clearly visible several times. And it's clearly shown that he is holding nothing in his hands or mouth. He's running fast, non-stop, and his mouth is wide open. Immediately following one of the cuts, a lit joint has suddenly appears in his mouth.
When Jerry escapes the country club by jumping through a window, the window has a set of brown slats open covering the inside of the window. When the shot changes to the outside with Jerry coming out through the window, the slats are now gone.
At the beginning of the film, Rebecca has her hair curled and sleek in a ponytail when she's in the office the first time, when she follows the two guys downstairs to the car her hair looks like it's been wet and dragged through a hedge for no reason whatsoever.
At the Deer Creek Country Club, the entire brawl takes place on the ground floor. Yet when Jerry escapes, he jumps through a window, tumbling over a roof what appears to be the first floor, landing on the pavement at the ground floor.

Incorrectly regarded as goofs

At the end of the movie, the group runs out of the hospital room playing tag. Earlier in the room you could see Jeremy Renner's character's wife was standing in the doorway, but in the shot immediately after you don't see her in the hallway.

But before the men run out of the hospital room, Jerry's wife is seen to step to the side of the doorway inside the room so she would not be visible in the next shot from the hallway as the men come out.

Revealing mistakes

When the golf carts are parked, before the chase scene, both of the windscreens only cover the bottom half off the front. However, when Jerry is dumping golf balls behind him, the tailing cart can be seen with a full windscreen to prevent the actors from being injured.
The scenes at The Sandpiper bar in Spokane were shot at North River Tavern in Sandy Springs, GA. When the taps at the bar are visible there are several breweries shown that are not available in Spokane.
In the forest scene, Jerry places a boom box on the ground in front of a tree and presses play on the cassette deck. "Crazy Train" by Ozzy Osbourne begins to play and can be heard throughout the forest, but the reels on the cassette never turn while the music is playing.

Plot holes

In the church scene Callahan is tagged by Hoagie and then later tagged Hoagie again even though there is a no tag back rule. After that Hoagie was "it" until he tagged Jerry.

Character error

At minute 46 in the YouTube version, Rebecca (Annabelle Wallis) tastes a Hors-d'oeuvre, then looks directly at the camera.
