Goofs

The driver of the SUV disappears after the accident and is never seen or mentioned again.

After the car accident, the patients have backpacks they are carrying, but in the subsequent scene after they arrive at Cade's mansion, the backpacks are gone.

When the mental patients are "trapped" in the SUV, it's clearly visible that the car windows are down and that the patients are just miming as if they are up.

When the police arrives at an alleyway after some explosions went off, the digital filter of flashing sirens does not cover the entire screen and just stops near the top.

There's a scene in the operating room at the mental hospital where a part of the doctor's hand is visibly floating in the air before he walks in.

Right after Dr. Kurtz says the line "Be careful, you fool." in the SUV, she is briefly seen to be outside the car and in front of a moving background for a few frames before it cuts to another shot.

In the prequel film, Twisted Pair, Cade and Cale's last name was Altair; In this film it is now Altier.
