Factual errors

When Ben transports to 1978, he meets Lieutenant Ellen Grier. At one point, she states she was number one in her class at the Academy, which would be impossible. The U.S. Service Academies did not accept women as Cadets and Midshipmen until 1976, so they didn't graduate until 1980.
This appears to be a Special Operations "mission". Women did not become eligible to attend The U.S. Army Special Forces Assessment and Selection (SFAS) course until 2013. In the last decade, few women were selected to attend SFAS, and not many of those went on to the SF Qualification Course. In the last decade, few women completed the Q Course, allowing them to wear the "Green Beret".
Pressure mines do not work as seen here. They detonate the moment sufficient pressure is applied. The notion that they don't detonate until the weight is lifted is a Hollywood fabrication.

After they open the crate and see the bricks you can see a man in the background who momentarily pops his head out.

Plot holes

In the flashbacks, Ian is shown to have taken it upon himself to input information about Ben and Addison into Ziggy to run scenarios about the probability of their success as a couple. In Judgment Day (2023), Ben told Addison that he'd asked Ian to do this.
The plan to save Bailey from the landmine requires that each member of the team take turns standing on the mine in descending order of weight. Ben lays out the plan as switching from Bailey to Ronny to Enock to himself to Grier. That is the descending order of weight based on Ben being there, but Ben has no way of knowing what his host body weighs, and this newer Quantum Leap series has established that Ben is inhabiting a host body, rather than swapping with it as Sam Beckett did in the original series.

Character error

This episode has an extremely high level of instances of Ben revealing details about his own life which may not be true of Perez, the person he is standing in for - especially referring to his fiancee - things the others could potentially know differently about as far as he knows, such that Ronny even questions and analyzes several. Not only is Ben highly intelligent and quick witted, he's been doing this for a while now (a whole season). This is extremely stupid and sloppy, and not believable with his intelligence and experience, doubly so after he's caught on this a few times.
