When José walks into the restaurant wearing his blue flight suit to tell his wife about being accepted for a space mission you can see both arm patches (US flag on the left arm and mission patch on the right), but after talking to his wife, when he's in the kitchen washing dishes, the right arm mission patch has disappeared.

Factual errors

Hernandez was not accepted into NASA until 2004.

In the movie he is already in training when the shuttle crashes, which occurred in 2003.

Furthermore he is then notified by the mission director that the Govt has approved new flights and he will be on the next shuttle flight, whereas there were 15 flights after the crash before he flew in 28 August 2009.

A massive ad lib by the script writers for dramatic effect.
Young Jose and his siblings watch Apollo 11 launch at what seems to be lunchtime. Apollo 11 launch took place at 9:32 AM EDT. Had Jose watched Apollo 11 launch live on TV it would have been 6:32 AM California local time.

What actually inspired Jose's own astronaut dream was live broadcast of Apollo 17's LEM lunar landing on 11th December 1972 at 11:55 AM California local time, which he watched on his black-and-white TV, holding the antenna to not lose reception, as depicted in the movie.
STS-128 was flown with space shuttle Discovery, but when the space shuttle is actually moved out of the vehicle assembly building the shuttle shown is the Atlantis.
During the launch of STS-128, Garret Dillahunt (as Rick Sturckow) is shown in the right-hand front seat on Discovery's flight-deck. As Commander, Sturckow was seated on the left.
In the scenes at Lawrence Livermore National Laboratory, no one is shown wearing a security badge. In fact, every person in a secure area at the Lab -- from the director to the janitor -- would be wearing a badge that has to be worn and visible above their waist. The badge indicates the wearer's security clearance level and to what areas of the lab they have access.

Incorrectly regarded as goofs

One moment, Jose is told he's not well, and will possibly be dropped from the program. Soon after, he's asked to be the chief engineer on a shuttle mission.

There are five significant scenes, between Struckland's berating of Hernandez as a potential washout and Hernandez being selected as Mission Specialist #2 on STS-128 Discovery. All of the scenes in this film are rather quick. The series of story events are neither soon nor sudden.

Revealing mistakes

The image of NASA's building shows the American flag hanging the wrong way. The Union (blue) should be in the upper left corner when hanging vertical. This seems to be a constant problem in Hollywood.
During the shuttle liftoff, Adela looks out the window, but there's no smoke trail. Then it reappears right on the next scene.
