Sex & Nudity

- Nudity is covered by a blur spot over a sim.
- Adult Sims are allowed to romantically interact with other sims, which also includes Woohoo.
- Some quest choices require to woohoo with a potential sim.
- Same-sex relationship are available, though doing that prevents the sim from having children with the partner.
Violence & Gore

- Fighting Heroes (Monarch, Knight and Spy) are able to threaten other sims for money, challenge to a duel and also kill them, if successful.
- Other Heroes engage into fistfights instead, which often happens while in the tavern.
- Sims can also die by starvation, disease, murdered by poison, or be executed.
- The fist fight is the least graphic from all fight types and it only leads to minor injuries.
- The sword fight is more intense than the fist fight, as they may get severely injured.
- The pit fight is (almost) always a fight to the death. While the fight itself is rather comedic, the deadly result is not. You can see the bloody tentacles from the pit monster while it eats a sim alive, leaving only bones behind.
- The physician treats the wounds of the sims and you see medical procedures, but it's not graphic.
- In the pirates and nobles expansion, you may fight a different style, which includes spitting into an eye or clubbing a stone into the other's face. Last one also leaves the Sim with a blue eye until the moodlet is over.
Profanity

- Sims may swear in occasional situations, but you don't understand them due to their different language.
- Some text boxes may become suggestive though.
Alcohol, Drugs & Smoking

- Sims may drink various types of alcohol like, wine, beer and ale. They also may drink a Drunk-Me-Not Potion to remove the drinking effects or the Mystic Grog to get instantly drunk (though not recommended).
- Smoking is not shown, but there is a reference to hemp.
- In the pirates and nobles expansion, you can also drink stronger alcoholic beverages and there is a reference about water pipes.
Frightening & Intense Scenes

- The Grim Reaper appears in all deadly situations and there is no more a comedic appearance like ghosts in the game.
- Any sim may be executed by the monarch.
- Compared to other Sims Titles, this is the most intense title of the series. However, due to the Fantasy-Setting, younger teens would still handle it.
