- It is Deb's first day as Lieutenant. She and Dexter get a text about a probable homicide.
We see that the jogger Travis abducted is held against his will. Travis says he won't be released until he repents to God.
The murder victim appears to be a prostitute who has been strangled and beaten, and has a chipped tooth. Quinn is dropped off at the scene (late) by a blonde with whom he obviously has spent the night.
Deb is looking at replacing herself on the force. Maria is very forceful in suggesting a candidate. Deb seems about to let Maria make the decision for her until Angel, watching the two women interact, calls her and tells her to stand her ground because if she allows Maria to impose her will on her, she will never be her own woman as a Lieutenant.
Dexter finds a notebook from his childhood when he was first tracking serial killers. The "Tooth Fairy" was a favorite, a killer known for murdering prostitutes in Oregon and removing a specific tooth. The prostitute found that day had the exact same tooth (lateral incisor) cracked. Harry reminds him that the tooth fairy would be over 70 years old by now.
Dexter stops by a retirement home near the crime scene. He pretends to be looking at a place to send his father and asks about residents from Oregon. He manages to find the name of one possibility, a Walter Kenney, who has been there for five years.
Masuka lets Ryan look at evidence from the ice truck killer case. She's particularly interested in a hooker's prosthetic hand.
During Deb's first briefing we learn the prostitute had some cocaine in her system. Dexter thinks about trying to connect it to the Tooth Fairy. Quinn and Deb argue about releasing information about the snakes to drum up a lead. Maria sides with Quinn and takes over the Deb's briefing.
Travis wants the jogger to tell God he's sorry. He apologizes and Travis leaves the room. In the other room Geller tells Travis that the jogger is just telling him what he wants to hear. Geller says the jogger has to be pure or "we can't proceed". Geller references picking Travis.
Dexter plays golf with Kenney (Ronny Cox). Kenney hurts his back during a swing and asks Dexter to take him to the club bar. Kenney says he was a pipe fitter for 25 years in Oregon. Dexter notices a dental implant in Kenney's lateral incisor. Sam calls Dexter to tell him his car is ready. Kenney asks Dexter to give him a ride the following day.
Sam tells Dexter they're having a company party coming up and he should come. It's a baptism of sorts for the guy Dexter helped save from the gang and he tells him he should bring his son. Sam thinks it would be good for Dexter to participate since he played a role.
Deb is freaking out about having too much on her plate. Dexter reminds her that she's new to the job. She says she doesn't regret breaking up with Quinn. Dexter finds that Kenney's mother was arrested for hitting her son in the face in public.
Dexter shows up at Kenney's house to pick him up. Kenney complains about his good-for-nothing son who won't return his calls. Dexter takes Kenney to fill a prescription and then gets him beer and porno magazines. The last stop is to a storage facility, where Kenney says Dexter can leave him. Dexter watches the old man go into one of the units and close the door.
Quinn tries to hit on Ryan and she shoots him down. Deb takes Quinn aside and asks how he's doing. Quinn is particularly bitter and accuses her of choosing her promotion over their relationship.
The jogger is able to escape briefly. He runs into a horse inside the church and Travis asks him to make a choice. The jogger falls at Travis's feet and begs for forgiveness. Geller tells Travis that he's ready.
Dexter waits until Kenney gets on a bus for home and goes into the storage unit. Inside he finds a box filled with teeth, confirming that Kenney is the killer that Dexter followed all those years ago. Dexter thinks about how he's going to kill Kenney (in his apartment) when the phone rings. It's Kenney, who says he's at a bus stop and doesn't know where he is. Dexter picks Kenney up. After a few moments Kenney pulls a gun on Dexter. He ran Dexter's plates after the pharmacy trip and found out that Dexter works for homicide. He tells Dexter he's going to kill him. Dexter intentionally crashes the car into a chain link fence which activates the air bags.
Masuka awkwardly asks Ryan on a date. She happily accepts.
Deb tells Maria she's going to go with a different guy to replace her as detective. Maria not only isn't supportive, but threatening because Deb isn't playing ball with her.
The guy Deb has selected because of his extensive homicide experience in Chicago, Mike Anderson (Billy Brown), shows up and initially is dismissive when Deb greets him. He assumes that Deb is kidding about being the Lieutenant as part of a hazing ritual. Deb very professionally and forcefully sets him straight, and he apologizes for his attitude, saying he had a tough plane ride.
We see that Dexter's plan worked and he now has Kenney secured on the slab of his "kill room" inside Kenney's retirement home apartment. Before Dexter kills Kenney, he takes a blood sample and Kenney realizes they are alike. The old man warns Dexter that this is what he has to look forward to. When he gets too old to kill, he'll be miserable. When Dexter realizes that Kenney wants his son to know he was the Tooth Fairy killer, Dexter chooses to smother him with a pillow instead to make it look as just another heart attack victim.
As Dexter talks about what's been happening we see Quinn having disinterested sex with another woman. We also see Ryan steal the Ice Truck Killer victim's prosthetic hand from evidence before going on the date with Masuka. When putting Kenney's blood slide away Dexter drops the entire box. He now has no idea which slide goes with which victim: There's no order anymore.
The episode ends with four horses trotting down a busy street. Each has a rider that seems to be a horrific combination of body parts from the jogger and a mannequin.
