Near the end of the movie, when the squad meet up, Lizzy can be seen in one shot in the front of Monster's car and in the next shot on the right of Sugar's car.
At around 46 mins, in the Murray house when Monster is telling Lizzy the she is not going out, the plat on his goatee is not there.
Just after Floyd gives John Wharton his gun, badge and credentials back, John drives his black Silverado pickup truck into a gravel parking lot. He backs in and parks with his front wheels turned on an angle, but in the next shot, his front wheels are straight.

Factual errors

During the final car chase Sugar fires his MP5 full-auto yet the weapon is clearly shown with the selector set on safe.

Revealing mistakes

In several shots of the final car chase with Sugar driving and Lizzy in the trunk, the stuntwoman's face is visible.

Miscellaneous

When the DEA team is in the training house and one of the members of the team forgot to check the corner with the dummy bad guy behind the door . The next scene the dummy is not hidden by the door as the door is opened outwards from that room .
At the end during the car chase tire tracks from previous takes can be seen when the car spins.

Crew or equipment visible

When the character who is shot during the money theft in the basement is picked up by another character picks him up, you can see the tube that supplied the fake blood.
When breacher opens the car door when he gets to the parking garage the camera and crew are reflected in it.

Plot holes

At the beginning of the movie Breacher blows up hundreds of millions worth of money in a pile, after DEA agents start to investigate they know there is 10 million gone missing seconds prior to blow up.
The team were asked where was the missing $10 million after the entire block of money was blown to kingdom come. No one in the entire world would know after such a devastation that there was any money missing because it was all destroyed and burned to ashes. The makers of the movie obviously didn't think this huge plot-hole through properly.
Since the money was blown up/burned; A: how do they KNOW that $10 million is missing, and B: whether or not there is some missing, the only ones who know are the cops. So, if the cartel finds out, guess who told them.
It made no sense for Breacher Wharton to tell the dying Lizzy in the presence of Detective Brentwood that it was in fact he who stole the $10 million. Breacher was suspected but never charged with stealing the cash, but now that Detective Brentwood knew about it, he immediately became a wanted man.
Breacher steals the $10 million in cartel drug money in order to have bribe money to track down the killer of his wife and son in Mexico. Yet instead of heading off to Mexico immediately with the cash, he needlessly stays behind with his drug enforcement team for an extended period as the vengeful cartels bump off the team members one by one, putting Breacher at serious risk of his life.
