When we first see the suicide bathtub, its inside is quite dirty but when Lance Preston sits in it, it's completely clean.
There is a big deal made of the fire escape that T.C. saw when doing the time lapse footage. Except that there are NO fire escapes shown on any of the buildings.
When Sasha does her EVP session right before her hair is touched, the voice coming from the tape recorder sounds nothing like what Sasha had just said.
It is revealed in the sequel that the cast of Grave Encounters used stage names as they were all actors: Lance's real name was Sean Rogerson, Sasha's real name was Ashley Gryzko, etc. However, when the crew wakes up with hospital bracelets, the bracelets are printed with their stage names instead. This is most likely because the producers never intended for there to be a sequel.

Revealing mistakes

At the very beginning of the movie, we see a summary of the episode of 'Grave Encounters'. But, at the end of the first sentence, the word Asylum is spelled wrong. The so-called episode is called: The Haunted Asyulm.
Possible deliberate error made by filmmakers: at first the suicide bathtub is quite dirty on the inside but when Lance Preston sits in it, it's clearly visible that it's completely clean now.

The worms in the cooler that lance opens are butter worms that are only native to Chile, they eat Trevoa leaves.

Anachronisms

As the history of the asylum is revealed, it is stated that a former doctor performed grisly experiments on the asylum patients in the 1930s. In a flashback sequence referring to this time, a patient is shown in front of a television, watching static. However, the television was not introduced to the public until the 1939 World's Fair, and was not common in US households until the early 1950s. An asylum in the 1930s could not, and would not, have had a television.
Near the beginning of the film they show footage from 1939-1943. It shows one of the patients watching TV which wasn't around at that time. TV was about 1955.
When, in the beginning of the movie they go with Kenny Sandavol into a room that has writing all over the walls. The writing is as crisp and clear as if it was done yesterday. It is stated that this hospital closed down in 1963. There is no way, even in the most pristine conditions, that the writing and the paint could look like that after 51 years! Especially in a room that has had no heat, A/C or any type of protection against cold and humidity. It would have only taken a year or two before the paint would have started badly peeling off the walls under those conditions. Yet, we are supposed to believe that the paint and the writing has been preserved so well fifty one years later.
