Continuity

During the roof scene, the patient is shown being intubated, then the Endotracheal tube is secured in place with a Bag-Valve Mask attached. A few seconds later, he is shown with just an oral airway in his mouth.
At the mass casualty incident after the explosion, the tanker is alternately on fire and then not on fire.

Factual errors

There is a scene where the Paramedics have a party near the Southern End of the Golden Gate Bridge. Their party is interrupted by a "Black and White" police car. The Southern End of the Golden Gate Bridge is Land belonging to the National Park Service. Park Rangers and US Park Police do not have "Black and White" police cruisers.
Numerous procedural errors during the mass casualty incident. The leaking fuel tanker should have been identified as a hazard before any patient care took place. Cardiac arrest is not treated during an MCI; the airway is opened, the patient is checked for a pulse. If there's no pulse and no respiration, they are dead and you move on to the next patient. Any fatalities on scene are left for the medical examiner; paramedics do not transport fatalities from the scene.

Anachronisms

During the roof scene, the patient is shown being intubated, then the Endotracheal tube is secured in place with a Bag-Valve Mask attached. A few seconds later, he is shown with just an oral airway in his mouth.

Errors in geography

The tanker crash is reported over the radio as occurring on "the Bay Bridge." The actual location is the 4th Street Off-ramp of interstate 280 North. The Bay Bridge is actually on Interstate 80.
