(at around 23 mins) When Light shows Mia the Death Note for the first time he asks her to read the last entry. The last entry should have been Anthony Skomal, not Kenny Doyle.
(at around 10 mins) When Light first writes in the Death Note, Ryuk hands him a pen and you hear the distinctive click of a retractable roller-ball pen. Cut to Light writing Kenny's name and he's using a fibretip fine-liner. Then when Light is writing the 'How' hes using a roller-ball again.
(at around 38 mins) When Detective James Turner first meets L in his office to discuss the case of Kira, he is handed an ice cream cone before he sits down. The cone is in his hand untouched for the first few shots before it eventually disappears from his hand when he is handed case folders.
(at around 1h 17 mins) When "L" is chasing Light Turner through the diner, he jumps up and runs along the counter. One can clearly see "L" slips and is about to fall on his butt; however, it switches to a different camera angle and he has not fallen but is still running.
(at around 16 mins) When Ryuk reads some rules to Light, you can see on the top of one of the rules he is reading, that there is the rule of if there is no cause written of the death, the person shall die of a heart attack. But later on, all deaths that they write have to have a cause or it will be as the dealer says.

Factual errors

(at around 28 mins) Light tells Mia that 'Kira' means light in Russian and Celtic. There is no such language as Celtic, rather there is a Celtic family of languages. The Extant Celtic languages are: Irish, Welsh, Scottish Gaelic, and Breton. 'Kira' doesn't mean light in any of these languages, in Irish for example the word is 'Solas'. Furthermore, most Celtic languages don't even use the letter 'K'.

Incorrectly regarded as goofs

(at around 48 mins) Ryuk says that nobody has been able to write his name in the Death Note, several times says that previous owners have only been able to write two of the four letters that make up his name, but when Light is reading the rules, the phrase "Ryuk is not to be trusted" is clearly seen. However, Ryuk is referring to someone writing his name to be killed, since what we saw was just a warning this is why the writer wasn't killed before he/she could finish.
(at around 48 mins) When Light threatens to write Ryuk's name in the notebook, Ryuk says that no one has written past the second letter in his name. Prior to this conversation Light read a note that said "Do not trust Ryuk, he is not your friend or pet" meaning Light should have known it wouldn't work on him and that someone had gotten past writing Ryuk's name. However, what we saw written was a warning not a notice of death, a Death Note of you will, so maybe there's different rules for that (there is a lot of rules after all) Plus who says Ryuk is his real name.

Revealing mistakes

(at around 1h 26 mins) On the airplane L is making a call, the phone screen clearly shows he is not.

Miscellaneous

At one point, a news report claims 6 death row prisoners wrote something on their cell walls in perfect Japanese. The next newscaster then states the message read "Lord Kira has returned to punish the wicked." However, that is not what the writing on the wall said, nor was it perfect Japanese. The actual message was "punishment for evil ... [missing verb here], god of a/the new world."
A crew member can be heard coughing in one scene.

Errors in geography

(at around 1h 19 mins) Near the end of the film as Light Turner is running towards the Seattle Great Wheel (or Ferris Wheel as they call it) there are city lights and skyscrapers across the water. The Seattle Great Wheel sits on a pier. From the angle of the shot the camera would be facing out into Puget Sound - and away from the Seattle skyline.
Most of the shots of the Seattle Great Wheel and the pier it sits on are not an accurate depiction of the pier. In the film there are many small food and souvenir stands along the long, narrow pier. The real pier is much shorter, much wider, and has large warehouse-like buildings housing various attractions and businesses which leave very little room for police vehicles to access the Great Wheel.

Plot holes

(at around 38 mins) L reasons that Kira is in Seattle because only that city saw the broadcast of the hostage situation. However, this reasoning ignores that the broadcast was also streamed online, as this is how Light and Mia found it, where anyone could have watched.
Mia tells Light that she has written his name in the notebook when got hold of it and the only way to undo that is to burn the page. Right after that, Light gets the notebook back. He could have just burned the page without having to devise an elaborate scheme with a Ferris wheel.
