(at around 1h 25 mins) During the final trap of the film the SWAT team saws through the door. From their perspective on the outside of the trap room, they start sawing through on the right hand side of the door but when the scene moves to inside of the room, the saw is also coming through from the right hand side of the door. It should be on the left hand side of the door when seen from inside the room.
(at around 16 mins) During playback of the first USB flash drive's video on Schenk's office computer, showing the red spiral pattern on a wall, the progress bar (length of video indicator) moving from left to right jumps to an earlier point after a scene change despite the dialogue continuing in the background.

Several detectives are carrying old Gen 2 Glocks, which is odd since Glock stopping making them in 1998. While not a mistake most large city police departments issue either the Gen 4 (2010) or Gen 5 (2017) version, as they have more safety features and are more reliable; the only cops that will still be allowed to carry an old Gen 2 would be ones that were on the force prior to 1998, known as "grandfathering". What is a mistake is the fact that Zeke's gun keeps changing from a Gen 2 Glock to a Gen 3. Compared to the Gen 2 the Gen 3 has a more textured grip with finger grooves, a redesigned extractor that has a loaded chamber indicator and an accessory rail.
(at around 1h 8 mins) When Marcus Banks (Samuel L. Jackson) is searching in the building, after he goes through the plastic curtains, his hands are all of a sudden bloody with no explanation why.
(at around 13 mins) As Detectives Zeke Banks and William Schenk walk along the subway platform to investigate their first murder, the camera and crew are visible in the train's windows on each passing.
(at around 38 mins) Detective Zeke Banks urges the on-duty Sergent to put the box received from delivery boy on his desk. The Sergent is wearing black surgical gloves. Detective Zeke Banks opens it immediately, bare-handed.
A veteran police officer would not keep a spare key on the frame above the door, that is the first place a criminal would look for it.
