When Devon Watson is first shown in the hospital he is wearing a traction splint on his leg. But when Dr. Benoit looks at his X-ray it shows tibia fibula fractures. Traction splints are only used on closed femur fractures with no other injuries to the leg, hips, pelvis, ankle, and or foot.
Both Jean and Tony highly suspect that the dead pedestrian is a drug mule with drugs in his body and that the sister and friend are both involved with the drugs. They would definitely call the LEO's to come investigate and to contact the suspects. Not only that, but the LEO working the auto-ped would have shown up at the hospital during his investigation.
In the opening scenes, Samantha Bailey portrays a little blonde girl holding a doll with angel wings ("the angel of death"). In the closing scenes, she is revealed to be just a "lost girl," but now has red hair. The lost little girl with red hair at the end is a different girl than the 'Angel of Death.' This is why the nurse Carly has the reaction she does, because it's not the same girl she saw.
At approximately 39 minutes, when Jeanne is finishing her exam of Tony and is forced to excise the drugs from her dead patient, the camera pans across to the corpse of the drug mule, Devon, and his foot clearly moves and twitches several times.
When Gibbs and McGee are talking about the 'Robotic Agent' and Gibbs says "It's like tracking a dirtbag McGee, you stake out his house... put out a BOLO... keep tabs on the girl..." Gibbs mouth isn't moving for the words "Put out a BOLO", though he picks up with "keep tabs on the girl." The audio for "Put out a BOLO" also gives away that it is added due to Gibb's speed and tone changing.
When McGee is explaining how he tracked the CIA's order for the polygraph tests, he mentions it went though Puerto Rico, but the map shows the Dominican Republic highlighted in red.
When McGee tells Gibbs that someone is monitoring his hacking, he says, "I had to shut down my UBT port." He probably meant to say "UDP", a common network protocol. The subtitles, which show "UPD", are closer but also wrong.
