After falling, the skateboarder is on his stomach spread eagle facing the ramp. In the next shot, he is on his stomach facing away from the ramp.
When in the Captain's office watching the security footage, the Captain notes that the security guard got off one round, but in the "Here's what happened" flashback, the guard clearly fires two shots.
Capt. Stottlemeyer punctures Lt. Disher's cup and Disher puts his finger on the hole to keep the drink from draining. The position of the hole in the cup changes repeatedly during the scene, and it is sometimes covered with the index finger, sometimes with the middle finger.

Factual errors

Randy figures out the type of Motorola phone that Tony Gamelobo (the robber who died of the heart attack) had, because he says he has the same model, but Motorola chargers charge all Motorola model phones so he can't have known that Gamelobo had the exact same model as him.
Looking at a map Monk notes that all even numbered roads are East-West. Not true. I-495 is East West.

Incorrectly regarded as goofs

The bulldozer pushes Troy's car down into the pit from the rear, then covers it with gravel from the side of the pit. When the camera pans out after the car is buried, there is a large pile of gravel behind where the back of the car would be. Therefore it would have been impossible to push the car in from the rear.

The large pile of gravel behind the car was pushed there by the bulldozer. You can see the path the machine made to push it there. (The bulldozer pushing the dirt there was not shown on screen.)

Revealing mistakes

Randy Disher says he drinks Diet Coke, but when Captain Stottlemeyer pokes a hole in his Styrofoam cup, it is a colorless liquid that flows out.

Errors in geography

The shot of the exterior of the bank is stock footage of the American Express Building, located at 65 Broadway, New York City.
When Natalie, Troy, Ridley, and Pez's hunt for the money, they stop by an ungated grade crossing. A train horn is heard, and a Metrolink passenger train passes through the crossing. Metrolink is the commuter railroad service for the city of Los Angeles. The geography of the surroundings in the scene is most likely to depict the area around Altamont Pass, where the Altamont Commuter Express (ACE) service runs.
When Monk is at the convenience store, there is a sign in the background with a 626 area code phone number. 626 is a Los Angeles area code, but the show is supposed to be set in San Francisco.
