Factual errors

(at around 30 mins) When Ted tells Jake the means of the first murder in 'the game' he says that Jake did it with "you mixed ethanol and nitric acid with the alcohol [sic]; probably a flavored alcohol to mask the bitter taste..." This is incorrect. Acids are well-known to be sour in nature (eg, lemons are sour due to citric acid); bases are bitter.

Even worse, the mixing of ethanol and nitric acid results in a reaction that gives off nitrogen dioxide, a toxic gas, and the reaction is so highly exothermic that it can result in an explosion. You can see what really happens by doing a quick search for the two ingredients on YouTube.

Revealing mistakes

(at around 50 mins) When the doctors slam the Asian man's corpse into the wall (on the gurney), it is clear that the actor puts his hands up defensively before he falls.

Plot holes

Character error

(at around 1h 21 mins) When Ted is performing Gwen's autopsy, the entire time the body block (under her neck) is left in place which would have made the autopsy difficult. Usually the pathologist will place it under the back, causing the arms and neck to fall backwards to stretch and push the chest forward to make it easier to cut open.
