Continuity

After the chase and shoot out, Simons car is missing his rear window and right tail light. Yet, when his "goods" get delivered the same day, the damage is all fixed. This, without Simon having the opportunity to let it get fixed.
During the long car chase, you can see that the car chasing Simon is a mid to late 1980's Lincoln Town Car. When the car crashes and flips, it is a redesigned early 1990's model Lincoln Town Car.
Seagal's Chrysler 300C in the movie has three sets of wheels. All of them appear to be OEM Chrysler, but are obviously three different sets. The first set have a chrome machined face and seven spokes. The second set have a cast appearance with six spokes. The third set are of a monobloc design, and in fact may actually be hubcaps.
When a special package is delivered to Simon, the delivery person goes into the store and Alice tells him Simon is in apartment "A." Later on, Armand and the East Side gang go to Simon's apartment to kill him. When one of the gang-members opens the door, you can clearly see the apartment is marked "#3"
When Simon gets shot at by the Morrison brothers, the first bullets hit the hood of the car. In all the next shots, the holes are gone.

Factual errors

In the shootout between Simon and Shaw, Shaw runs out of bullets, symbolized by a "click". However, he is shooting with a pistol. When a pistol runs out of bullets, the slide remains in its backwards position, for faster reloading. When it is in that position, the pistol can never produce a click.
When Simon is looking up the gangsters in the police database, their addresses have the ZIP (postal) code 56556. All California zip codes start with 9. The 56556 code is McIntosh, Minnesota, about 2000 miles (3000km) away.

Revealing mistakes

Throughout the movie several different cars can be seen mounted with the same license plate 2PCE275.

Audio/visual unsynchronised

While Simon and Gary are riding in Simon's car after the run-in with the skinheads, Gary mutters the word "Shit", and immediately afterwards, Simon's lips are moving without any sound being heard.

Crew or equipment visible

During the extremely long car chase scene, in one shot, towards the end of the scene, you are able to see Steven Seagal's stunt driver wearing sunglasses.

Character error

At the beginning of he film, Steven Seagal goes to the police desk and introduces himself as Simon Bannister. From then on, he is known as Simon Ballister.
