During the opening montage, we see Michael boarding a tricycle-gear aircraft, but the aircraft seen flying out is a DC-3, a tail-dragger.

Revealing mistakes

The opening sequences show what is presumably Michael's bank record. The shot shows a screen dated last quarter 2004. The series went to air in 2007 and is presumably supposed to be current time with screening time. The bank shot is therefore of details about three years earlier than when he was burned.

Plot holes

One major aspect of the show is that it often involves CIA missions. The CIA is prohibited from any domestic operations, and therefore so many episodes' plot revolving around routine domestic missions would be entirely impossible.

Character error

Throughout the series, Michael pulls into places whenever he parks so that he must back out when he leaves. With his background and training, he would "combat park," meaning he would back in so that he could pull straight out to leave. This is done in order to drive away quickly in a life-or-death situation.
Despite being a former SF operator and highly trained intelligence operative, Michael Weston often incorrectly holds handguns, using the "teacup method" of control, meaning that he rests the weapon upon his palm while firing it.
