After Leon has his tongue ripped off, you can see the tongue intact in his mouth.
There are regular continuity problems with blood spatter. In the opening scene, we see a clean train car, then suddenly one of the doors and walls is splattered (this blood couldn't have come from splatter when the man fell). When the man slipped in the blood, we see a puddle extending down a few chair rows, but when the camera pulls back up there is almost no blood on the floor. At 53 minutes in, we see large blood spurts during an attack, including some getting on a window, but then when the camera changes positions there is no blood on the train car walls.
In the opening scene, when the passenger wakes, there are four seats on his left parallel to him and the length of the train, then two at a 90 degree angle, then the doors, then two more at the same 90 degree angle. He looks in that direction (left), gets up, and walks in the same direction, then slips on a large puddle of blood. When he falls, his body from the waist up is between the first two 90 degree angled seats and the last two parallel seats, with the puddle of blood extending at least as far as both of the parallel seats. Even a man groggy from sleep would have seen that much blood from the very first time he looked in that direction, and would have avoided walking in that direction, and this would be true even if he thought it was oil (the blood appears black in the light).
When Leon and Jurgis are speaking in the art gallery just before meeting with Susan Hoff, there is a painting in the background between them, being hung. After they move away from where they were standing to speak to Hoff, the same painting is hanging on a different wall. Of course, since this is art, it is possible that they are merely very similar and are two of a series.

Factual errors

The Leica M, while being a great compact camera, can not take the pictures it is shown to take in this film. It would need a zoom lens to do that.
When the main character is hiding from the butcher in the metro you can see as sticker warning to leave the space for the door clear. The translation in Spanish in the same sticker is word by word and completely senseless.
Hitting the back of the head with a meat mallet won't knock the eyes out.
The subway cars in the train have signs above the doors saying "Green Line". in actual NY subways that space is occupied by advertising. Placards with train names are above windows in the middle of cars, and NYC Transit never identifies trains by color. The public refers to them as such due to the color of their routes on the official NYC Transit subway map.
The cop in the subway grabs Leon's camera and says since they are on an Orange Alert (on the terrorism scale) he can do what he wants. This is absolutely false. A person filming from a public area (which includes subway stations) is allowed to do so and no police officer can legally seize their camera without consent or probable cause of a crime. Filming in public is not a crime nor is it probable cause of a crime.

Revealing mistakes

After the Driver rips out his tongue and turns to slaughter Maya, Leon cries out "No!" The "n" sound cannot be pronounced by someone lacking a tongue.

Miscellaneous

The title says it's a midnight train, but when confronted, Detective Hadley tells Maya it's the 2 o'clock train.

Anachronisms

When Leon is showing Maya the newspaper article dated December 19, 1895, a closeup of the newspaper shows a column of copy containing the words, "bikini-clad babes and tanned hunks". Putting aside the unlikelihood of that style of news-writing in 1895, the term "bikini", as regards clothing, was not coined until the mid-1940's.

Crew or equipment visible

When Maya is searching through Mahogany's bathroom, at one point the white t-shirt of a crew member is visible in the mirror.
In the subway car at the opening of the film (approx 00:02:20), a ladder is visible through the left window, highlighting the use of a set and studio.

Errors in geography

When Maya boards the train to find the Butcher, it is marked "Grant Station". There's no such station in the NYC subway. There is a Grant Avenue, but that would always be identified as such, without "station", on official signage.
The security gate on the meatpacking plant is marked "141 HERRON ST" There is no such street in New York City.

Plot holes

After the second murders, Mahogany is shown cutting some sort of boils or nodules off his chest and saving them in a jar. No reason is ever provided for the source of these nodules, why they are on his chest or why they are cut off and saved.

Character error

Near the end of the film where Maya goes to the subway, she gets through the gate by scanning her card on the wrong part of the barrier.
Detective Hadley tells Leon the girl he rescued was a celebrity model. A celebrity model would not ride the New York subway alone at 2 AM.
