When arriving at the Y Class planet they wish to shelter on, Janeway gives commands to the crew to prepare the ship for landing. She orders "red alert," but she should have ordered "blue alert" for a planetary landing. However, the lights on the bridge do reflect a blue alert condition.
As Tom and B'Elanna leave their wedding, there is already rice on the ground from previous takes, before people start throwing rice.
When Tom brings B'Elanna to sickbay, she has her uniform jacket on one arm but when she was in her quarters prior, she had removed the jacket and undershirt and was in her tank top when Tom found her.
When Tom visits B'Elanna in sickbay, from her point of view, he is holding a device with the honeymoon holo-program in his right hand. When she is seen from slightly behind Tom , the device is in his left hand.

Factual errors

The crew are duplicated by giving samples of their DNA. The real Doctor is a Hologram, therefore no DNA means the Doctor would not have been duplicated.

Incorrectly regarded as goofs

Towards the end, Captain Janeway says, "Mr. Paris, resume course." To which Lt. Paris replies, "Aye, Sir." It was established early on in the series that Janeway is addressed by the crew as "Captain" or "Ma'am in a crunch." Paris would never intentionally address her as "Sir". Paris has always been a bit of a rebel, so he might very well call the captain "Sir."

Plot holes

Duplicated Tuvok states that any object brought aboard duplicated Voyager 'within the past 30-40 weeks' is immune to the cellular degradation problem. Duplicated Neelix shows examples of these objects, including vegetables, a metal keg of alcohol, ore samples, and particle accelerators. Yet when the real Voyager finds the degradated remains of the duplicated ship, there is no trace of the immune objects, which should have remained somewhat intact, even after being exposed to space.
The Biomimetric Voyager crew should not have an actual ship in which to travel in. The prefix Bio belongs to the word Biological. The only parts of the ship are the Biometric Gel Packs. The ship itself is not Biological.
Neelix says the replicated items are also affected. This is nonsense as unless the energy used by the replicator first forms matter composed of silver blood which then mimics the desired object it will be made out of the correct immune atoms.

This would then also clue the crew in to the fact they're fakes long before the mystery illness spreads.

Character error

When Tom is in sickbay with B'Elanna telling her about their honeymoon, he tells her it is for 6 days and 7 nights. The correct expression is 6 nights and 7 days.
