Dr. Weir is told by Dr. Fletcher (Alan Ruck) that the car that killed Simon impacted the driver's side door. (at around 15 mins) Dr. Weir views a news report on a computer terminal. This includes a picture of the accident showing that the car was impacted on the passenger's side, judging by the clear view of the back seat (no airbags) while the car is on its side.

Incorrectly regarded as goofs

During the psychiatric hospital scenes, Weir is seen wearing a robe that has a sash. Normally this would not be permitted out of fear she would hang herself. However, the scenes in which Weir was in hospital were part of her dream as she lay fighting off the Replicator attack on her body. They were not supposed to depict an actual hospital nor were they supposed to depict proper hospital procedure.

Character error

Although Dr. Weir is in the hospital for multiple days, she never reacted to the fact Dr. Fletcher always wore the same tie, which, along with the Jack O'Neill glitch, should have added to the realization it was not 'reality'.
