In Ripple Effect (2006), Sam states that the trip through the stargate averages around 0.3 seconds, and that a duration of 3.4 seconds indicates a "significantly longer trip". In this episode, she corrects Cam when he says the trip took 3 seconds by clarifying that the trip took 3.2 seconds.
At about 31 minute, when Teal'c is hiding around the corner with a small group of SFs, two Ba'al clones are seen walking down the hallway. When the scene switches, there is only one, then a second enters the hallway calling him back.
At approximately 32 minutes, Ba'al #6 is checking a corridor and you can see fragments of another following behind him. At the scene cut, the one behind is missing as he turns from checking the same corridor. Then Ba'al #5 comes into the scene at the end of the hallway.

Revealing mistakes

At 31:35, when one of the Ba'al clones brings Carter into the room, the face of another Ba'al clone is visible over his shoulder who is clearly not Cliff Simon, the actor who plays Ba'al.

Crew or equipment visible

When the SUV arrives at the crash site and the team step out, the camera, crane, & crew are all visible in a reflection on the door side panel.

Plot holes

The Ba'als keep using access cards to get into various areas. Since the SGC knows who was captured, it should have been simple to cancel the cards, thus denying Ba'al access to anywhere.
