When Dean is talking to Kate on her balcony he holds her head with his right hand. Whenever the camera view is from behind him the gold bracelet is missing. When it's from behind Kate, the bracelet is shown on his wrist.
At the end the view of the Bay behind Stone shows the wake of a large ship in the water behind him. When the camera view switches to Keller and his prisoner, the water is clear.
In the final scene, Dean asks Kate to visit the summit of the Golden Gate Bridge overlook (probably Marin Headlands) to watch the sunset. At first Kate declines, but eventually consents. However, when they arrive at the overlook, there is no sunset. In fact, the weather is clearly overcast, which is not a surprise given San Francisco's unpredictable coastline.
When Stone and Keller visit Dean Knox's place of work, his fiancée comes storming out of his office. Next to its door on the right hand side are windows. Once Stone talks to Knox, and there are more close-up shots, instead of windows there's suddenly a solid wall separating the offices.

Revealing mistakes

When Martin Sheen is robbing the bank and hits the bank lady in the head with his briefcase, he hits her on the right side of the head. Later, she is seen with a band-aid on the left side of her face.
When Stone and Keller are reviewing the films from the bank surveillance camera, they are somehow able to pan with the characters, but also zoom in from a different perspective.

Crew or equipment visible

During the chase scene near the end of Act 3 the reflection of the camera inside of Kate Evans car could be seen in the drivers side window.

Errors in geography

Although seen in the series a number of times, the view from Mike Stone's San Francisco Police Headquarters office window is a wing of the Los Angeles City Hall. It is particularly obvious in "Betrayed" where a portion of the famous tower is also visible.
