When Neelix and Kes are looking for Kes' quarters, her locket disappears and reappears several times.

Factual errors

Voyager is encountering a spatial distortion in the shape of a ring, which the crew can't figure out how to escape. In space, though, they have the option of movement in all three dimensions, so they could have merely moved the ship along the z-axis in order to evade the distortion. It would have been better to have the distortion in the shape of a hollow sphere.
Marseille does not take an 's' after the last 'e'.

Incorrectly regarded as goofs

When B Elanna Torres is entering the transporter room (in hopes of finding engineering) you can see a door to the left which is actually a part of the engineering section. However once B Elanna is in the transporter room, the door disappears. Ordinarily, this would be considered a continuity error, but since we are explicitly told that the phenomenon that the ship has encountered is continuously rearranging the layout of the ship, we may assume that the door's disappearance was caused by the phenomenon.

Character error

When Ensign Kim runs into Lieutenant Baxter in the corridor Kim addresses Baxter and talks to him as if they are of equal rank, Baxter even asks Kim if he should report to his station, which is something an officer would never ask of someone that they outrank.
