Tuvix

Star Trek: Voyager

Factual errors

The Doctor explains that doctors used radioactive barium to image the lining of the intestines. Regular, non-radioactive, barium is used. Barium compounds absorb X-Rays allowing the intestines to be imaged. Radioactive properties are not needed and would be dangerous.

Incorrectly regarded as goofs

Though he is the more senior officer, Lieutenant Hogan addresses Ensign Kim as 'Sir'. This is because whilst Hogan is senior in rank, Kim is a Bridge Officer and part of the command crew. In this instance, Hogan is addressing Kim's seniority by position not by the rank on his collar.

This has been long held in terrestrial armies and navies for centuries up into the current times. An Officer In Command (OIC) is deferred to even though their actual held rank may be lesser. It is done to keep the command structure of the vessel intact.
Janeway wants Paris to take a shuttle down to collect plant samples. Paris recommends waiting until morning to avoid bad weather. He could just as easily land on the other side of the planet where it is daylight; however, the plant samples Janeway requires may be indigenous to the area in which Paris is to be sent and not elsewhere.

Revealing mistakes

Towards the end when Tuvix goes to sit down on a bed in sickbay, a leftover triangle of orange tape (used to mark actor placement during rehearsals) can be seen on the bed before he sits down (and is then gone in subsequent shots).
