Kes senses Harry in imminent danger when she is in Sickbay assisting the doctor. The doctor calls the bridge, and B'Elanna transports the away team back. Janeway then walks back to her command chair in time to catch Kes half-fainting when 8472 talks to her again telepathically. Somehow, Kes makes it from Sickbay to the Bridge in just a minute and a half.

Factual errors

After learning that something terminated the power signatures of the Borg armada Janeway decides to investigate and orders Paris to lay in a course for their position at warp 2. Harry stated they were 5.2 light-years away, it has been established that warp factor 2 is 10 times the speed of light, hence it would have taken Voyager 6.24 months to travel a distance of 5.2 light-years at warp 2.

Incorrectly regarded as goofs

When Janeway and Chakotay are in her ready room talking about the problem they are facing regarding the Borg, it's said "Three years ago I didn't know your name, today I can't imagine a day without you." This is clearly a mistake because in the very first season it's known that Captain Janeway does indeed know Chakotay's name because she is talking with Tom Paris about the mission she was ordered on. Also in the 5th season show Relativity (1999) Admiral Patterson says Chakotay's name in the ready room as well when asked about Tuvok making contact with Starfleet. However, Janeway is clearly referring to the time just before she was sent on the Maquis mission, which would have been roughly about three years ago.

Revealing mistakes

When Janeway orders Chakotay to take an away team to the Borg ship, she tells him that they will keep an open com link and an active transporter lock on them. When Chakotay is on the Borg ship, he needs to tap his communicator and request a connection with Captain Janeway. This would not be necessary if they had an open com link. Later Torres can not get a lock on the away team to transport them. She should have known the lock was down previously if she was actually keeping an active lock on the away team.
Species 8472 has weapons that can destroy a large number of Borg ships, yet whenever they fire on Voyager, there is minimal damage and at times the shields holding.

Miscellaneous

At 12:11 the mock-up of a control panel has a typo: "STARFLEET DATABSE."
When Chakotay is telling Janeway of "The Scorpion and the Fox" he refers to it as a parable. Parables specifically exclude animals. This would be a Beast tale, a type of fable. Also, this fable usually features a frog instead of a fox.
While on the Borg cube, Tuvok tells Chakotay and Harry to lower their phasers so they will not appear to be a threat. However, while they continue exploring, everyone still has their phasers raised.

Plot holes

When 15 Borg ships pass Voyager, they create subspace turbulence that destabilizes Voyager's warp field and causes the ship to drop out of warp.

The Borg vessels are shown approaching Voyager from behind and are moving at sub-light speed (evidenced by how they are visible passing Voyager once it has dropped out of warp). Voyager had been moving at warp (faster-than-light) speed. There would be no way for ships moving at sub-light speed to have overtaken a ship moving at warp.
