Star Trek: Voyager

Continuity

Tuvok tells the four Maquis that he has cleared Deck Thirteen of personnel for their 10K run, and later tells them that he increased the gravity on that deck by ten percent, yet once they finally reach Deck Thirteen, they pass several crew members, and none of the people they pass seem to notice the increased gravity.

Incorrectly regarded as goofs

Despite the fact that the four Maquis malcontents are supposed to be refusing to follow Starfleet protocols, when Chakotay first confronts them, Dalby addresses Chakotay by his Starfleet rank of commander, rather than by name. This can be explained by the fact that Chakotay, himself, was one of the Maquis and Dalby served with him, meaning that he remembered the respect he had for Chakotay.

Revealing mistakes

When Ensign Kim reported all communications are down, Captain Janeway immediately calls Engineering over the com system. If communications were down at that moment she shouldn't have been able to contact Lieutenant Torres.
Kim reports to Janeway that all communications are down, yet she is able to call Torres in engineering. A moment later, when Tuvok and his team are trapped in the cargo bay, he calls the bridge to find out the situation, but cannot get through.

Miscellaneous

Tuvok makes Crewman Gerron remove his earring, but Gerron is a Bajoran and the earring is part of his culture. In previous Star Trek series Bajoran star-fleet member are shown wearing them.
In "Caretaker" B'Elanna and Chakotay were the only Maquis crew members seen on their small ship. Yet here, three other never before seen Maquis members are trained by Tuvok, members who never participated in finding their lost crew mates.
