When Tom hugs B'Elanna after hearing about the death of her friends, B'Elanna's hands keep changing position as the camera angle changes back and forth.
Captain Janeway notes that it is not her custom to send an away mission of one, which Tuvok later tells Seven is recommended Starfleet procedure. However, she has done so on three previous occasions: Chakotay in Initiations (1995) and Nemesis (1997) and Harry Kim in Non Sequitur (1995).
When Voyager first picks up the Starfleet transmission, Kim determines it is coming from a relay station 3.8 light years away. The crew makes their way there, stopping halfway due to gravitational waves coming from the relay station, Janeway states it is still two light years away. Voyager then approaches the station and Janeway says that Voyager has been travelling "at high warp for two days" to get there. To get just a couple of light years? Voyager could get that far in minutes at high warp.
When Seven enters the Ready Room to propose an Away Mission, the camera or crew are reflected on the shiny door that she walks passed.
