The defense grid surrounding the planet Teresia was originally described as a polaron grid, but Commander Chakotay later refers to it as a Tachyon grid when telling Captain Janeway that they were beginning to poke holes in it.

Revealing mistakes

As Harry Kim is led into a bedroom, there is green tape visible on the floor used to indicate where Garrett Wang should stand.

Plot holes

The gag that Harry uses on the girl is a simple strip of fabric that does not cover her mouth. While it would have kept her from speaking clearly, she would have had no trouble screaming or making noise.
No one says 'cancel red alert' (or anything to that effect). Voyager returns to normal status on its own as soon as Tuvok says the Nasari ship is moving away.
When the Taresians are explaining to Janeway, Kim, Paris, and Tuvok about how Kim was a member of their race, they have very specific knowledge of the part of space Voyager is from, their home world, and how he could have been born all before they even described where they are from in the first place. Therefore, they shouldn't have any knowledge of this information.

Character error

Although it is standard policy for female Starfleet officers to be called "sir," Janeway doesn't like it and has told her crew not to follow that policy. Near the end of the story, Chakotay calls her "sir."
