The whole Voyager crew, marooned on a planet, is first shown comprising only about 30 individuals; later the number appears to grow closer to 60. Still, this is far short of any official numbers for the crew, which vary around 140 to 160.
When two Kazon shuttles attack Voyager 22:08 into the episode, the ship is at warp while firing on the Kazon vessels; however, while at warp the nacelles are in the down position instead of the usually angled warp position.
When Voyager arrives at the origin of the Kazon distress call, Janeway orders Red Alert. She advises Tuvok to remain at Red Alert as she goes to sickbay with Chakotay. Upon arriving in sickbay, the ship is no longer at Red Alert.
Lieutenant Commander Tuvok states that a Kazon ship approaches off the starboard bow of the U.S.S. Voyager; however, the Kazon ship approaches off the port bow.
Left stranded on the planet, one of the primary concerns of the Voyager crew is finding drinking water. Even as Janeway tells the crew to find said water, the mountains in the background are visibly snow-covered.

Factual errors

When the Doctor is accidentally materialized in space using the holoprojectors on the hull, he's able to talk and speak. Being that space is a vacuum he shouldn't be able to speak at all even if he is a hologram.

Revealing mistakes

When the holograms of the Talaxian ships are projected into space they are hit by weapons fire from the Kazon ships. They being holograms means that any directed energy should pass through them.
When the Kazon were searching the ship for crew members the massive discharge was shown to be on the same side of the ship that Lon Suder was on. However the damage to the doors and hallway was on the opposite side of where the discharge occurred.

Miscellaneous

When Voyager is almost getting ready to intercept the Kazon ships Tuvok says the distance between Voyager and the vessels. When he gets to the 3,000 mark he says just 3 however the closed captioning says 3,000. Clearly an error in what he has said.

Plot holes

Character error

At approximate run-time 12:42, Janeway misspeaks her order to Torres. Instead of saying "Work with the Doctor on it", she says "Work on the Doctor with it".
