After Wesley sets the phaser to automatically fire repeatedly on the force field, the Main Bridge is alerted to his presence on Deck 6 and dispatches security teams to that deck. However, in the next shot, Wesley is walking towards a turbo lift which is labeled as being on Deck 7, not Deck 6.
Wesley's small hand phaser on the wooden hallway railing, has the phaser firing slightly to the right, but, when he sees it firing at a wider angle shot, at the force field, it's firing all the way to the right corner, indicating a completely different positioning from the one he left it at.
The "baby hairs" (thin wispy hairs along the hairline) on Ensign Leffler keep appearing and disappearing, indicating that Ashley Judd's hair was styled with them smoothed back or left loose on different filming days for consecutive scenes.

Incorrectly regarded as goofs

Since the game uses a visual interface, and works through the eye, and optic nerve, Geordi should have been immune to its affects, since his eyes and optic nerves don't work, and the game could not have been altered to work through his VISOR. However, the game is sending information to the brain, via the optic nerve. The VISOR 'sees' the same visible portion of the spectrum as others' eyes and sends the information to his brain. There is therefore no reason why the device shouldn't affect him too.
Wesley Crusher says to Data, shortly after his arrival, that he thought he was on the wrong ship. Data makes artificial laughter and says "Uhh" while his eyebrows are changing shape like he is really seeking for the right words. This is typical human behavior, which Data often emulates in his quest to become more human.

Revealing mistakes

A matte painting is used to extend the section of Jeffrey's tube. however, the painting was created so that it creating a vanishing effect when viewed straight on, and was then filmed at an angle. Because it's a flat image with no depth, the illusion does not work at all.
The back of ensign Leffler's collar is horribly misshapen, coming down to an inverted point at the very rear.

Miscellaneous

In the third act, when Wesley and Robin are sitting in 10 Forward having dinner, a fly lands on Wesley's plate when Robin says, "If we worked together we could figure it out."

Plot holes

Robin tells Wesley that she needs to get to engineering right away or Geordi will wonder why she's late. But it's already assumed that she and Wesley are under the power of the game-force so he would in fact understand perfectly why she wasn't there.
After freeing them from the Game's control via a series of flashing lights from a palm beacon, Data explains to the bridge crew that he has programmed every screen and terminal in the ship to emit the same light pattern, which should free the rest of the crew as well. Data could have programmed the screens and terminals on the bridge in the same way, saving him the time it took to travel there personally and use the handheld beacon when he knew the ship was in danger and time was of the essence.

Character error

At time index 5:32, Picard informs Riker that Wesley will be joining the Enterprise via shuttle for vacation from the academy. At 9:16, Wesley arrives via an Oberth class starship, not a shuttle.
Wesley suggests to Robin that law 103 should be "A couple of light years can't keep good friends apart." Considering how she grew up, it's much more likely this law would be among the first few.
Picard tells Riker he has good news, but then informs him that Wesley Crusher is coming for a visit.
