Data says "The Bortas has lost its port shields. It is unlikely they can withstand another hit in that quarter." A moment later the Bortas is hit three times on the port side. It is clear that its shields are still intact.

Incorrectly regarded as goofs

Worf grabs the ritual blade with his hand. When he removes the hand, red blood is left on the blade. Although Klingons have purple blood in Star Trek VI: The Undiscovered Country (1991), they have red blood in all other Star Trek (1966) productions before and after that, so it is that movie that is in error, not this TV show.

Revealing mistakes

When Kurn arrives to assist Gowron's ship, he drives off the attacking Bird-of-Prey until it cloaks and flees. Klingon ships cannot maintain shields or fire weapons while cloaked, yet the disruptor fire from Kurn's ship is seen impacting the shield bubble of the fleeing ship even as that ship has already begun to fade from view.

Miscellaneous

Right after Gowron has been installed as leader of the Empire, as he is walking toward Worf to give him back his honor, in the lower right hand section of the screen you can see Picard throw his hands up (you can only see one hand though) in a "What the" moment.

Plot holes

Gowron petitions Picard for the Federation's assistance against the Duras uprising, who denies his petition. The matter is then dropped. Picard is the captain of a single Starfleet vessel, no matter how prestigious a posting to the Enterprise might be. There are many layers of leadership above him, from admirals all the way up to the president of the Federation. Gowron, and Worf, could have gone over Picard's head and petitioned the actual Starfleet or Federation leadership.

Character error

Gowron states that women may not hold seats on the Klingon High Council yet offered K'Ehleyr a seat on the council in Reunion (1990), and women were shown on the High Council in Sins of The Father (1990).
When the members of the Enterprise gave honor to Worf as he went to the transporter to join the Klingons, everyone was standing at attention except Troi. who was standing At Ease. While Troi wore unique uniforms which was part of her job as counselor, she was still a Lt. Commander. As such, she should have been at attention since Picard gave the order.
