Revealing mistakes

Footage of General Martok's ship docking at Deep Space Nine was often recycled for each time a Klingon Bird of Prey visited the station. However, it is stated numerous times that Martok commands one of the smaller Birds of Prey (known in publications as the B'Rel Class) while other Klingon ships (known as the K'Vort class) are huge Bird of Prey vessels, about the same as a Galaxy class starship, and which would need to dock on one of the space station pylons due to their larger size.
As the crew of the Rotarran are having dinner in the mess hall, Jadzia grabs one of the Klingon officers and tosses him off his seat before pinning him to the floor. As Jadzia is holding the Klingon officer on the floor, we see she is only wearing one gold pip and one black pip on her collar instead of two gold pips and one black one (for her rank of Lt. Commander).
When the Rotarran is at warp, some of the streaking stars in the background can be seen passing through the ship.
Martok gives the order for the Rotarran to get underway in Klingonese ("Mahk-cha," Klingonese for "engage"). As all the dialogue is presumably in Klingonese, just translated to English for the audience's sake, it makes no sense that this one word would go untranslated.
After breaking up the fight in the mess hall, Dax's uniform is still clean, but in the next scene, there is blood on the gray shoulder yoke. She says it came from taking Ortakin to surgery. Given the injuries Ortakin was shown to have sustained, there's virtually no way that blood would have ended up on top of Jadzia's shoulders even if she had to carry him to the infirmary herself. Obviously, blood was smeared on that portion of her uniform because it wouldn't show up on the black of the rest of the uniform.

Plot holes

No explanation is given as to why Martok, a respected general who was assigned as liaison to the Federation by Chancellor Gowron himself, would be given command of a derelict and dishonorable crew and ship like the Rotarran to carry out an important mission.
While Worf is on detached duty aboard Martok's ship, his duties are split between O'Brien, Bashir, and Dax. Worf is a relatively new addition to the crew (only having been on DS9 about a year and a half by this point). Any duties he had were previously taken care of without burdening the rest of the senior staff. there is no reason to do so now.
The Rotarran finds the B'Moth, which has been adrift for several days emitting a distress signal that can be detected from at least 7 light years away. Given that Jem'Hadar patrols are known to be in the area, it would be virtually impossible that the B'Moth wouldn't have been found by them first.
The Rotarran cloaks after detecting a Jem'Hadar fighter nearby. The Dominion is known to be technologically superior to any race in the Alpha Quadrant. If the Jem'Hadar were within the Rotarran's sensor range, the Rotarran would have been well within the Jem'Hadar's before they were able to cloak. Yet the Jem'Hadar did not detect the Rotarran.
Worf enters into his combat log that the Rotarran has "reached the Cardassian border and begun our search for the B'Moth." They received a distress signal, they would have the B'Moth's location. No searching should be required.

Character error

Worf's detached leave seems to put a burden on the rest of the staff, with Bashir, O'Brien, and Dax all having to pitch in to cover for him. Why then was Dax also allowed leave at the same time? Kira had already worked out a duty roster reliant on Dax being present due to her security clearance. Letting Dax go on Martok's ship with Worf would put further burden on the remaining members of the senior staff.
While fighting Martok on the bridge, Worf swings widely at him and missing, spinning a full rotation before facing Martok again. A trained fighter like Worf would have much better control of their movements and know to not make such a maneuver as it would leave his back exposed to attack.
The Rotarran receives a distress signal from the B'Moth, yet no attempt is made at that time to establish two-way communication to assess the B'moth's status and verify the authenticity of the message.
After Martok has defeated Worf, the tactical report comes in stating that a Jem'Hadar fighter is on its way and will be within weapons range in 9 minutes. The Rotarran then turns to head towards the Jem'Hadar, rather than carrying out its rescue mission. 9 minutes would be ample time to beam the B'Moth's survivors, who ended up numbering only 35, aboard the Rotarran and make an escape, engaging the Jem'Hadar only if necessary.
Martok instructs Major Kira to have twelve barrels of bloodwine waiting for his crew in the airlock when they dock to celebrate their victory. Why is he telling Kira this. Acquiring provisions for his crew is not within the scope of her duties, nor is it an area she is known to excel at. He'd be better off contacting Quark.
