When Adriana is playing darts with Tony, the two darts she throws hit the board in the bottom half and the middle of the board, but when she gets them, she pulls the darts from the upper right part of the board.
After Tony snorts a line of cocaine, the remaining line on the mirror and the plastic straw next to it change positions a couple times between shots.
When Christopher confronts Tony about the accident, Tony is wearing a tan sport coat, a yellow shirt, dark pants, and brown shoes. At least one full day passes, as evidenced by the change in Tony's clothes to his short-sleeved shirt. Later, when you see Tony in Dr. Melfi's office he's wearing the sport coat outfit again.
In the car accident scene, just after the accident when the truck starts to steam, the rear marker lights go out in the close-up. In the long shot after that, the marker lights are on again.
During the accident scene the car slowly swerves and stops, then suddenly accelerates and jumps and turns upside down. Physically it makes no sense for the car to accelerate and jump like that after it came to an almost full stop after the swerve.

Revealing mistakes

When Christopher shoots Tony's car at the Bing, his first shot makes two bullet holes a few inches apart because one of the squibs went off prematurely.
When Christopher is shooting at the maroon Chevy Suburban in the Bada-Bing's parking lot, the slide on the semi-automatic pistol that he is using never cycles, indicating that it is a prop gun.
When Christopher is shooting at the maroon Chevy Suburban in the Bada-Bing's parking lot and "shoots out" the left rear tire, you can see that the pistol is pointed well above the tire itself.

Crew or equipment visible

When Christopher is telling Adriana about his toothache, he opens the door to his truck. In the truck mirror, for a split second, you can see a crew member wearing a gray hat, and another crew member standing next to him in a black hat.

Errors in geography

Christopher says I-95 is a straight shot from New Jersey to Raleigh. I-95 doesn't go to Raleigh.
Christopher tells his men to meet him in Winston-Salem, NC to load up on cigarettes (presumably untaxed). Winston-Salem was then still the site of manufacturing operations for the R.J. Reynolds Tobacco Co., which makes the Winston, Salem, and Camel brands of cigarettes. However, when he arrives back in New Jersey and opens up the van, there are several boxes marked "Marlboro," a brand manufactured by Philip Morris, mainly in Richmond, VA.
