Steve has nothing with him when he first starts running towards the villain's compound, yet as he nears the plant, he is wearing a side-pack with an over-the-opposite-shoulder strap.
Black Shoes Turn White: When Steve runs toward the bad guy's industrial compound, he is wearing black shoes, and even leaps over the camera-- giving the audience a close-up view of his feet. But in all the next scenes in the segment he is wearing white shoes.
Steve pours full glasses of OJ for himself and Jaime, yet when he raises his glass to drink from it, the glass is nearly empty.

Revealing mistakes

When Jaime flashes back to crushing the glass of O.J., she is using her left hand, but it's her right hand that is bionic.
The tennis court sign clearly has no metal mesh behind the middle of it where Jaime's flung tennis racquet passes through it, obviously showing that the fence's chain-link fabric was cut out behind the sign to allow the racquet to break through the sign more easily.
When the limo pulls away with Rudy and Michael waving, there is clearly nobody in the back seat.
When Steve Austin and Jaime Sommers get shown the photo of their mission adversary, and get told his name would be Carlton Harris, the back of the photo has the name of the actor (Dennis Patrick) printed on it.

Plot holes

The two large red switches are each wired to explode the vault door if both switches aren't turned at the same moment, yet nothing happens when Steve turns one of the switches.

Character error

The armory's two security switches have to be thrown at the same moment, yet Steve and Jaime don't bother to synchronize their watches before Jaime heads off down the hill toward the villain's compound.
