When Adrian is on the phone with his doctor's answering service, Darwin is pulling out cups and bowls from Adrian's cabinet, including a blue bowl. After the camera cuts away to a close-up of Adrian, however, the bowl is once again on the shelf.
In the summation, when Ian Blackburn gets shot, his glasses fall to the floor. A couple of shots later, after Ian's been shot the fourth and final time, his glasses are back on.
At the beginning, Benjy got into a fight at school and received a scrape in the middle of his forehead. But a couple of days later in the story when Disher is arresting Sharona, he has neither a bandage nor an injury on his forehead.
A chimpanzee is wrecking Monk's apartment. A picture is seen slanting to the left, but you see an umbrella is hanging from the right side. The picture should be leaning to the right because of that.
When Monk reaches for the phone in his kitchen, the cabinet door is closed. Seconds later when Darwin jumps to that counter, the cabinet door is open.

Factual errors

Throughout the series, Adrian Monk has been shown to be highly educated and a perfectionist. However, when he first sees the chimpanzee (an ape), he inaccurately calls it a monkey. Disher (of all people) corrects him. Throughout the episode, Stottlemeyer continually referred to the ape as as a "monkey." That loose terminology is to be expected from him. However, even if Monk hadn't known this (which is highly unlikely), he would have known better once Disher had corrected him and would correct anyone who called Darwin a monkey. He himself did so repeatedly. So did Disher, who had earlier corrected Monk. The only person who consistently corrected those who said 'monkey' was Sharona.
When the Captain is testing the chimp to see if he could fire a gun, he uses a Beretta with a light trigger pull, but the chimp was found holding a revolver which would have a heavier trigger, and would be harder to shoot. The Beretta would not be an accurate test to see if he could shoot the revolver.
Sharona Fleming tells Adrian Monk that all he has to do to get her out of jail is to prove that Darwin is innocent. But that would only clear Darwin. Sharona, however, would still be charged with breaking and entering at the animal shelter when she stole him.

Incorrectly regarded as goofs

When the Captain is explaining how Ian built and got into the panic room through "the furnace," it is clearly a fridge. The fridge is seen once, opened by Monk, and then twice more after the crime is solved.

The Captain says they found the entrance to the tunnel in the basement behind the furnace. He was not referring to the fridge in the panic room.
Monk is talking to Dr. Kroger on the phone when Darwin pulls the base of the phone off the wall. As he does so, it can be seen that there is no cord connecting the phone to the wall jack.

A lot of wall phones were designed to plug directly into the wall jack without the use of a phone cord.
There is no mention of the murder weapon being checked for fingerprints.

Had the gun been tested Darwin would have been excluded and the killer included. However, Darwin was seen with the gun in his hand which made everyone assume he pulled the trigger.

Revealing mistakes

The door to the panic room is made to look as if it's made of steel. But when a hole is cut into the door, and the inside of the door is visible, the door is obviously made out of wood.

Miscellaneous

Animal control picks up Darwin from the crime scene and takes him to the shelter to be "put to sleep." In the very next scene, Darwin is back at the police station with Monk and Sharona.
