The elevator door for Laszlo's private elevator opens to the right in the lobby, but opens to the left on Laszlo's floor.
Willie has a helper, Erickson, throughout the episode, but in the final scene when the team leaves, he is not in the car leaving with the team.
After Dunson shoots Laszlo, he is shown on the floor with his arms over his chest. But when Dunson moves to access the "secret room", Laszlo's arms are seen at the sides of his body.
When Jim inserts the 8 track tape into the player to get his instructions, the machine immediately starts playing without him turning the ignition key to provide power to it.

Revealing mistakes

There is a distinct line already visible in the brick wall where the fake wall slides down before it is lowered into position, probably from previous takes.
All of the floors leading to all of the elevators are continuous, revealing that the elevators are not real as there would be no way for them to move.

Miscellaneous

It seems far fetched that a man of Laszlo's obvious intellect and awareness, fails to notice the incongruity of the fake wall being closer than before when he opens the false safe.
Robert Ritchie's character is listed as "Erickson", but Willie refers to him as "Eric", and "Ericson" is the name of their fictitious air conditioning company.
It is not explained or shown how Willie and his helper Erickson, wearing uniforms of the Erickson Air Conditioning Company, are able to pinpoint the exact location of Laszlo's wall safe in order to lower a false wall with ample room for Barney to work.

Plot holes

After Barney gets the formula Willy's helper pulls the fake wall up through the slit in the ceiling yet Barney is not shown removing the fake safe that protrudes from the back of the wall.
When Laszlo opens the fake safe the dial finally stops in the number 5. But when Barney sees it, the dial is in the number 9, leading him to conclude that Laszlo made an additional turn.
