Peter Graves in Mission: Impossible (1966)

Goofs

Shape-Up

Mission: Impossible

Edit

Continuity

The tape player that Mr. Phelps puts the tape into is different from the one in which the tape self destructs, as the label on the front of the first one is in better condition. It is probable that they used two players so that they could pipe in smoke for the 'destruct' effect.

Factual errors

In the staged accident at the beginning, the blond fellow pushes a crate off a stack to hit the forklift driver below. The crate is marked 510 lbs. net weight, which would make it impossible for one man to slide it off the top of a stack by himself, even with the white "slick pad" seen under the crate.

Incorrectly regarded as goofs

The name of the "ship of death" Orion (pronounced uh-RYE-un) is pronounced "OH-ree-on" by several characters.

Both are actually correct. The first is the way it's pronounced by someone who is of American or British ancestry. The second, the one that the OP said was incorrect, is the way someone who is of Greek or Dutch ancestry would pronounce it.

Revealing mistakes

When the camera cuts to Morgan's dead body, you can clearly see that he is still breathing.

Crew or equipment visible

Toward the end, as Delany is walking up the gangplank of the Orion, a camera on a crane can be seen in the extreme far left of the frame.
Lt. Bill Orcott (Lonny Chapman) confronts Frank Delaney (Gerald S. O'Loughlin) for the first time as Delaney is getting into his car on the docks. After they speak for a moment as Delaney sits in his car, Delaney drives away and three large round studio lights and a member of the crew can be seen reflected in the passenger side of Delaney's car.
