When Willie knocks Shields out during their sparring session, his gi comes undone, revealing his chest and stomach. In the next shot, however, it is neatly tucked into his belt.
The external shots of Pantheon Studios office building pan up and zoom in on an office with two small side-by-side windows, one with an air conditioner unit. The internal shots of supposedly the same office has a wall to wall, ceiling to floor window.

Factual errors

When Jim is listening to the mission tape the switch is set to the record position. This would result in no sound being heard and the tape being erased as it ran.

Revealing mistakes

At minute 22:15 a Red Shirt straight from "Star Trek TOS" is worn by an extra entering the studio gate. Filmed nearly 4 years after CBS cut that series, it seems the costumes were still around also.
Willy gives Theo Dane a shot to make him appear dead. There is a close up of the syringe which is clearly marked "Live Virus".
