When talking to the little girl, Olivia mutes the polycom speaker phone device as the girl recites a rhyme so she can argue with captain Cragen. The LEDs on the polycom speaker phone device change from Green to Red to indicate the phone has been muted. In the next few seconds, Cragen makes a statement about how the traffic sounds are looped in the background and the as the camera follows him as he moves closer to press a button, the polycom is shown to be illuminated in green (unmuted). In the following seconds Olivia is shown unmuting the phone, turning the LEDs from red back to green.
At about 11 minutes in, Olivia mutes the speaker, to talk to Cragen. She does not unmute it, when she next speaks to Maria, but Maria hears her, & answers her.

Factual errors

Maria tells Olivia that when she lived with her mother in Honduras, she had a cat named Chimichanga ("You know, like the food," she says). Chimichangas are an American invention, and while they can sometimes be found in northern parts of Mexico, they wouldn't exist in Honduras.
When Richard was attempting to drive away from the crime scene and is stopped by Benson. He stops the car and Fin punches him and forces him out of the car. Richard doesn't put the car in park. If a car wasn't in park, it would've moved on its own.

Revealing mistakes

When doing CPR and mouth to mouth on the little girl in the yard she briefly has her eyes wide open in one shot and has her eyes closed in the next.

Errors in geography

When Det. Tutuola is at the site of the burnt-out restaurant said to be at 342 94th Street, a camera shot captures a street sign for West 127th St.
