Factual errors

A courtroom scene takes place on Monday, September 5th. That would be Labor Day, a Federal holiday, and court would not be in session.
Jack McCoy would have been in a lot of trouble for allowing Mrs. Capetti to lie on the stand.
When McCoy and Kincaide are arguing about what to let Mrs. Gaines plea to Jack says "I want her to do time Claire!" and Claire replies "manslaughter carries a mandatory prison sentence" to which Jack scoffs "yeah a whole two years." This is not true, manslaughter in the first degree is a violent, class B felony in New York and carries a mandatory minimum sentence of five years in prison. That number is not negotiable, the law requires that someone convicted of a violent, class B felony serve at least five years in prison, it also states they cannot serve more than 25 years in prison.

Plot holes

Character error

The psychiatrist at the Gardner Clinic makes multiple errors, and possibly commits crimes, by revealing confidential medical information about his former clients to Lenny and Rey. According to medical-privacy laws, the psychiatrist isn't even allowed to confirm whether a client is under his care, let alone what the client confided to the psychiatrist in therapy.

Moreover, the psychiatrist reveals that two of his clients had an affair, and this fact is used by the police to expand their investigation. Both clients of the psychiatrist would most definitely have any convictions against them thrown out because the evidence against them was illegally obtained. Patient-client privilege is absolute, and without the psychiatrist breaking of confidentiality, the police would never have had a case.
When Det. Curtis asks Claire if she's okay after the shooting, he calls her "Caitlyn."
