Incorrectly regarded as goofs

McGarrett arrives at the scene where Tobias died, He checks the step that broke when Tobias stepped on it, which was the third step. Che Fong tells McGarrett it was the "sixth step"; a few minutes later Danny refers to it correctly as the "third step".

Che Fong tells McGarrett that he has a cotton fiber from a nail that came from the sixth step. If Tobias crashed and fell through the third step, it stands to reason he fell downwards toward the sixth step and caught his pant leg on a nail in the sixth step as he fell, then hit the deck before falling in the water, as they described it.

Revealing mistakes

When McGarrett visits State Sen. Bolin to discuss the Tobias death the previous night, Bolin slams a copy of the Honolulu Advertiser on his desk that purportedly has a large cover story questioning whether Tobias' death is related to a murder trial the year before. One can clearly see where the two photographs and the phony headline have been cropped over other, unrelated stories from the real Advertiser using blank white strips of paper.
The signed guest check by Ray Tobias shows 4 items that add up to $11.60. Yet the check total is $11.00. Takes place at 17:43.

Crew or equipment visible

When Chin and Danny are escorting jewelry designer Makoto to their office, the camera shows a close-up of the right rear corner of their car as they walk around the rear of the vehicle. The boom mic is reflected in the area above the "Custom" logo and disappears as they move, while a crew member who is smoking a pipe is also visible.
