After removing the man from the ice house, Johnny and Roy both ride in the ambulance, with Chet following in the squad. When leaving the hospital to return to the station, Chet is no longer there, just Johnny and Roy.
When Captain Stanley, Chet and Marco enter the icehouse, they are all wearing air masks and carrying two extra tanks for Johnny and Roy. But when they reach Johnny and Roy, the extra tanks have disappeared.
When they load the fall victim onto the boom lift his face is covered up and feet uncovered with the yellow blanket. When they unload him on the ground his face is uncovered and feet covered.
After the patient is removed outside of the ice house, the right arm clothing is dry even though it was frozen to the ice and they sprayed water on it.

Factual errors

When Roy calls in to Rampart about the retired sea captain who glued his hands to his model boat, Dr. Morten says that the glue sounds like a "cynoacrylic", and tells Roy that it is very dangerous and the government recalled it the year before. Cyanoacrylic glues are in fact in very common household use to this day. We know them as "superglue". Dr. Morton reads later that the glue can be easily removed with acetone. Acetone is the main component in nail polish remover, so the paramedics could probably have removed the glue on site.
When Johnny and Roy are ventilating the little boy, the first thing they should have done is remove his pillow to properly align his airway for effective ventilation.

Revealing mistakes

After the frozen victim is brought into Rampart, Dr. Morton inserts a Tracheal Intubation into the patient, but the Laryngoscope is not inserted in the mouth but rests instead on the side of victim's face.
When the ammonia leak is recognized inside the ice house Cpt. Stanley goes out to get air tanks and masks, but allows the door to close. He should have known to prop the door open to allow ventilation for Johnny and Roy, who were still inside with the victim without their masks and air tanks.

Plot holes

Incorrectly Regarded As Goof. When Chet drives the Squad to the hospital for Roy & John, he is not seen at the hospital afterwards. He could have gotten a ride back to the firehouse from another paramedic team that had been at Rampart around the same time or another scenario that viewers are not informed of could have played out.
