Continuity

When Jack points his gun at Nina's head the hammer is in the uncocked position, even though he had just fired two shots. Which means the hammer would be cocked, Jack was not shown uncocking it.

Factual errors

When Reza's mother arrives at the Warner's house, she is wearing a traditional Muslim headscarf (hijab). After she learns that Reza is being interrogated, she sits down and we can see that she has removed her hijab. The whole point of wearing the hijab is that a traditional Muslim woman will not have hear head uncovered in public. (Some might argue that perhaps Reza's parents are more liberal Muslims, but if that were the case, then his mother would not have been wearing the hijab in the first place.)

Character error

While they were transferring Nina, Ed shouldn't have told Jack what kind of security CTU set up for them in front of her.
Roger Stanton, the director of the National Security Agency, is wearing civilian clothing and is often addressed as "Mr. Stanton" by others. However the director of the NSA is required to be a commissioned flag officer in the United States military and upon appointment is promoted to the grade of four-star admiral or general depending on the branch they serve in. Stanton should be wearing a military uniform, and the proper way for subordinates and piers to address him is either as Director Stanton or by his military rank.
